In order to gather the data needed to meet the scope of our plan for the Research Registry for Neonatal Lupus, your continued participation is vital.

Communication with you is scheduled to continue until September 29, 2011.  Follow-up questionnaires will be generated from our database and mailed to registry participants at 6 month intervals.  The information you supply in these follow-up questionnaires is very important.

As we review the database, we may need to contact you to clarify information.  It is essential that we have a current address and phone number for you at all times.
